Twenty years ago penile girth enlargement was performed by removing fat cells and injecting them under the skin of the penis.  Fat, like any tissue removed from the body, starts to die as soon as it loses its blood supply.  Soon after being transplanted to the penis, a new blood supply would develop, but it was not reliable or predictable, and by the time it was established, many of the cells had already died.  A lot of the fat, sometimes most, or even all of it, would be reabsorbed, and the fat that survived was sometimes distributed unevenly.  

In the late 1990's, we started using AlloDerm, which is material processed from donor skin.  All cells are removed from the material so there is no possible transmission of disease or rejection. After it is implanted, your own cells grow into it as it becomes your own tissue.  Since AlloDerm is not living tissue, immediate establishment of a new blood supply is not critical so the long term results are more predictable.

We are now able to get excellent results not only with AlloDerm, but also with fat injection. Through the use of fat stem cells, the process of revascularization is greatly enhanced, so there is better survival of the fat.  Fat is removed and placed in the penis the same as before, but it is processed in a way that separates stem cells from the rest of the fat, which are then injected into the fat in a very high concentration. A new blood supply is established rapidly which allows for a much better result.  There is a small amount of reabsorption, but nothing compared to what we used to experience.  There will be an increase in circumference of 30-40% either way.
